OCT PRIVACY AND RETENTION POLICY
OCT retains student records for seven years. OCT does not sell or provide operator information to anyone without written authorization from the requesting operator. Upon written request, OCT Academy will release permanent record information to the operator's State Department of Health Services, Operator Certification Unit and/or the named Operator's Utility of employment. All information is the sole property of OCT Academy and is strictly confidential.
Cancellation Policy

If you cancel an agreed upon contract for educational session(s) for any reason prior to the contract confirmation date, you will be liable for the following charges:
- If you cancel after the calendar date(s) have been confirmed, you will owe OCT, Inc. 50% of the face amount of the agreed upon contract amount.
- If you cancel within two (2) weeks of the dates scheduled, then you will owe 75% of the face amount of the agreed upon contract.
Upon cancellation, the amount of the cancellation charges will be determined and a billing sent to you, which will specify the amounts that will be due and payable within 30 days of the date of the billing notice. Any deposit required under this agreement shall be forfeited and applied to the billing amount assessed. Failure to pay the net billed amount within the 30 day period will result in all legal collection remedies being used to collect the debt, including reporting under the Fair Debt Collection Act to credit reporting agencies. In addition, any unpaid and assessed monies due us will be charged the legal rate of interest under the Federal Regulation Z (as amended) of 1.5% per diem.
Should OCT, Inc. be required to seek collection of the debt owed, then any Court of competent Jurisdiction, or assigned arbitrator by the Court, shall award attorney fees and costs to the prevailing party therein.
Upon entering into a contract or accepting a proposal with OCT, Inc., an Authorized Officer or Agent of the Organization must read and sign a copy of the preceding cancellation notice.

Refund Policy

For Self-Study Training Materials: Refund for our products will be given (less a 20% restocking fee) if the product is in mint-resale condition. Exceptions are CDs and Videos, unless they are defective. Replacement copies will be sent for these two items. All customers returning items will be charged a 20% restocking fee, responsible for all shipping and handling fees, must enclose the original invoice with the return. All returns are allowed up to 30 days from the date of purchase.
For Training Sessions: A refund will be made up to 48 hours prior to the first program day, less a $75.00 administrative charge. Cancellations within 48 hours of first program day are subject to full course fee.
